Introdução: os estágios de mineralização dentária têm sido utilizados em diversos estudos para a estimativa de idade, apresentando confiabilidade nos resultados. Objetivo: correlacionar a mineralização dos terceiros molares e a idade cronológica pelos métodos de Nolla, Demirjian e Nicodemo, Moraes e Médici Filho numa amostra populacional do nordeste brasileiro. Métodos: de um universo de 1 205 radiografias panorâmicas digitais, foram selecionadas 297 imagens de pacientes atendidos rotineiramente em um serviço de Radiologia Odontológica privado. Todas as imagens foram avaliadas em ambiente escurecido por três examinadores calibrados, utilizando as classificações de Nolla (1960), Demirjian (1973) e Nicodemo, Moraes e Médici Filho (1974). Os dados numéricos foram resumidos através das estatísticas descritivas de locação e dispersão. Para análise estatística foi ajustado um modelo de regressão linear simples considerando a idade como variável resposta. Para os testes, foi adotado o nível de significância de 5 porcento ciento. Resultados: o desenvolvimento dos terceiros molares foi mais adiantado no sexo masculino na maioria dos estágios, para os três métodos. Nos dentes superiores, o dimorfismo sexual foi mais evidente no estágio 7 de Nolla (diferença de 16,34 m no dente 18 e 14,43 m no dente 28). Nos dentes inferiores, as maiores diferenças ocorreram: no estágio 6 de Nolla (diferença de 13,92 m no 38 e 10,46 m no 48) e estágio 5 de Nicodemo (diferença de 16,23 m mais elevado no elemento 38 e 9,9 m no 48). O desenvolvimento dos dentes terceiros molares dos lados direito e esquerdo apresentou-se semelhante, contudo os dentes superiores apresentaram-se mais adiantados que os inferiores. O presente estudo permitiu o desenvolvimento de fórmulas baseadas em regressões lineares simples utilizando os três métodos propostos, possibilitando, assim, estimar a idade cronológica dos indivíduos da amostra. Conclusão: houve correlação do desenvolvimento dentário dos terceiros molares com a idade cronológica pelos três métodos(AU)


Introducción: las etapas de mineralización de los dientes se han utilizado en varios estudios para estimar la edad, lo cual ha mostrado resultados fiables. Objetivo: correlacionar la mineralización de los terceros molares y la edad cronológica por los métodos de Nolla, Demirjian y Nicodemo, Moraes y Medici Filho en una muestra de población del nordeste de Brasil. Métodos: a partir de un universo de 1 205 radiografías panorámicas digitales, se seleccionaron 297 imágenes de pacientes tratados de forma rutinaria en un servicio de radiología dental privada. Todas las imágenes fueron evaluadas en cuarto oscuro por tres examinadores calibrados, utilizando los métodos de Nolla (1960), Demirjian (1973) y Nicodemo, Moraes y Medici Filho (1974). Los datos numéricos se resumieron utilizando estadística descriptiva del contrato de arrendamiento y la dispersión. El análisis estadístico se ajustó un simple modelo de regresión lineal, considerando la edad como variable de respuesta. Para las pruebas, se adoptó el nivel de significación del 5 pr ciento. Resultados: el desarrollo de los terceros molares fue más temprano en los varones en la mayoría de las etapas, por los tres métodos. En los dientes superiores, el dimorfismo sexual fue más evidente en la etapa 7 de Nolla (diferencia de 16,34 m en el diente 18 y 14,43 m en el diente 28). En los dientes inferiores, las mayores diferencias ocurrieron: en la etapa 6 de Nolla (diferencia de 13,92 m no 38 y 10,46 m no 48) y etapa 5 de Nicodemo (diferencia de 16,23 m más elevado en el elemento 38 y 9,9 m no 48). El desarrollo de los dientes terceros molares de los lados derecho e izquierdo se presentó semejante, sin embargo los dientes superiores se presentaron más adelantados que los inferiores. Este estudio permitieron el desarrollo de formulaciones a base de regresiones lineales usando los tres métodos propuestos, permitiendo estimar la edad cronológica de los individuos en la muestra. Conclusiones: se observa una correlación de desarrollo de los dientes terceros molares con la edad cronológica por tres métodos(AU)


Introduction: dental mineralization stages have been used in several studies to estimate age, and the results obtained have been found to be reliable. Objective: correlate third molar mineralization and chronological age using the methods developed by Nolla, Demirjian, and Nicodemo, Moraes and Medici Filho in a population sample from Northeast Brazil. Methods: of a total 1 205 digital panoramic radiographs, 297 were selected of patients routinely seen in a private dental radiology service. All the images underwent darkroom evaluation by three qualified examiners using the methods developed by Nolla (1960), Demirjian (1973), and Nicodemo, Moraes and Medici Filho (1974). Numerical data were summarized by location and dispersion descriptive statistics. For statistical analysis a simple linear regression model was adjusted which considered age as response variable. A significance level of 5 percent was adopted for the tests. Results: third molar development occurred earlier in males in most stages by the three methods. In upper teeth, sexual dimorphism was more evident in Nolla stage 7 (difference of 16.34 m in tooth 18 and 14.43 m in tooth 28). In lower teeth, the greatest differences occurred in Nolla stage 6 (difference of 13.92 m in 38 and 10.46 m in 48) and Nicodemo stage 5 (difference of 16.23 m higher in tooth 38 and 9.9 m in 48). Third molar development was similar on the right and left sides, but greater in upper teeth than in lower teeth. The study allowed development of linear regression formulas based on the three methods proposed, making it possible to estimate the chronological age of the individuals in the sample. Conclusions: the three methods found a correlation between third molar development and chronological age(AU)

As pigmentações negras do esmalte se devem à coloração extrínseca e estão associadas com problemas estéticos. O objetivo é apresentar dois casos clínicos de crianças com pigmentações extrínsecas negras do esmalte dentário, com ênfase no tipo de tratamento e no acompanhamento. Trata-se de um paciente de 5 anos de idade que utilizava constantemente sulfato ferroso para tratamento de anemia por deficiência de ferro; e outro paciente de 12 anos de idade com diabetes mellitus tipo 1 e dermatomiosite. No primeiro caso observou-se a presença de pigmentações negras extrínsecas nos dentes decíduos e nos primeiros molares permanentes, principalmente nas superfícies lingual e vestibular na região cervical e terço médio, além de lesões de cárie incipientes nas superfícies oclusais dos molares e manchas brancas ativas nos primeiros molares permanentes. No segundo caso, havia pigmentações negras extrínsecas na região cervical das superfícies lisas dos dentes decíduos e permanentes. No primeiro caso, foi realizado tratamento para remoção das pigmentações negras e polimento dos dentes por meio de profilaxia profissional com creme dental abrasivo que contém pedra pomes microgranulada. Houve uma melhora no aspecto clínico a partir da segunda sessão e finalizado na quinta sessão. Para o caso 2, observou-se dificuldade na remoção de manchas pigmentadas na região cervical e risco de sangramento gengival. Uma vez que a criança necessitava de antibiótico profilático, optamos pelo controle diário de biofilme e não remoção pela profilaxia profissional, uma vez que o paciente não se queixava do impacto estético. Conclui-se que as pigmentações negras extrínsecas podem ser observadas facilmente em crianças, e ainda que haja um comprometimento estético, não traz danos à manutenção da saúde bucal. Cabe ressaltar que não está claro como sua presença na superfície dentária reduz a suscetibilidade à cárie. Além disso, a escolha do tratamento sempre deverá ser baseada no risco-benefício para o paciente(AU)


Dark enamel pigments are due to extrinsic staining and associated with aesthetic problems. The objective is to present two cases of children with black extrinsic pigmentation in the tooth enamel, with emphasis on the type of treatment and monitoring. This is a 5-year-old patient of who constantly used ferrous sulfate to treat anemia by iron deficiency, and another 12-year-old patient with type 1 diabetes and dermatomyositis. In the first case, we observed the presence of black extrinsic pigments in the primary teeth and the first permanent molars, especially in the lingual and vestibular part in the cervical region and middle third, together with incipient caries lesions on the occlusal surfaces of molars and active white spots on the first permanent molars. In the second case, there were strange black pigmentation in the cervical region of the smooth surfaces of the primary and permanent teeth. In the first case, the treatment performed was to remove black pigments and polishing the teeth by means of professional prophylaxis and using an abrasive toothpaste containing micro-granules of pumice. There was an improvement in the clinical aspect from the second session and the treatment ended at the fifth session. For the second case, we observed difficulty to remove the pigmented spots in the cervical region, together with the risk of bleeding gums. When the child needed prophylactic antibiotics, we opted for the daily control of the biofilm and it was not removed by professional cleaning, because the patient did not complain about the aesthetic impact. As conclusions, the extrinsic black pigments can be easily observed in children, and despite its aesthetic effect, it does damage oral health. It should be noted that it is unclear how their presence on the tooth surface reduces susceptibility to caries or tooth decay. In addition, the choice of treatment should always be based on the relation risk-benefit for the patient(AU)


Los pigmentos de esmalte negro se deben a la tinción extrínseca y están asociados con problemas estéticos. El objetivo es presentar dos casos clínicos de niños con pigmentaciones extrínsecas negras del esmalte dental, con énfasis en el tipo de tratamiento y el seguimiento. Se trata de un paciente de 5 años de edad, que utilizaba constantemente sulfato ferroso para el tratamiento de la anemia por deficiencia de hierro, y otro paciente de 12 años de edad con diabetes mellitus tipo 1 y dermatomiositis. En el primer caso se observó la presencia de pigmentos extrínsecos negros en los dientes primarios y los primeros molares permanentes, principalmente en la parte lingual y vestibular en la región cervical y tercio medio; además de lesiones de caries incipientes en las superficies oclusales de los molares y manchas blancas activas en los primeros molares permanentes. En el segundo caso, había pigmentaciones negras extrañas en la región cervical de las superficies lisas de los dientes primarios y permanentes. En el caso 1, se realizó el tratamiento para eliminar los pigmentos negros y el pulido de los dientes por medio de una profilaxis profesional con crema dental abrasiva que contiene microgránulos de piedra pómez. Hubo una mejora en el aspecto clínico a partir de la segunda sesión y se finalizó en la quinta sesión. Para el caso 2, se observó dificultad en la eliminación de manchas pigmentadas en la región cervical y el riesgo de sangrado de las encías. Una vez que el niño necesitaba antibióticos profilácticos, optamos por el control diario de la biopelícula y no se eliminó por la limpieza profesional, ya que el paciente no se quejaba del impacto estético. Se concluye que los pigmentos negros extrínsecos pueden observarse fácilmente en los niños, y aunque haya un efecto estético, no hace daños al mantenimiento de la salud bucal. Cabe señalar que no está claro cómo su presencia en la superficie del diente reduce susceptibilidad a la caries. Además, la elección del tratamiento siempre se debe basar en el beneficio-riesgo para el paciente(AU)

AIM: Identify whether the degree of oral motor performance is related to the presence of teeth grinding and maximal bite force values in children with spastic cerebral palsy. METHODS: Ninety-five spastic cerebral palsy children with and without teeth grinding, according to caregivers' reports, were submitted to a comprehensive oral motor performance evaluation during the feeding process using the Oral Motor Assessment Scale. Maximal bite force was measured using an electronic gnathodynamometer. RESULTS: The teeth grinding group (n = 42) was younger, used anticonvulsant drugs, and was more frequently classified within the subfunctional oral motor performance category. Teeth grinding subfunctional spastic cerebral palsy children presented lower values of maximal bite force. The functional groups showing the presence or absence of teeth grinding presented higher values of maximal bite force compared with the subfunctional groups. CONCLUSION: In spastic cerebral palsy children, teeth grinding is associated with the worse oral motor performance.

BACKGROUND: Salivary osmolality reflects the hydration status of individuals with cerebral palsy (CP) necessary for an adequate unstimulated salivary flow rate. AIM: To investigate whether salivary osmolality could serve as a potential indicator of caries risk in children with spastic CP by displaying a stronger association with caries occurrence than salivary flow rate. DESIGN: The convenience sample consisted of 65 children with CP aged 6-13 years old. Unstimulated whole saliva was collected using cotton roll, and salivary osmolality was measured using a freezing point depression osmometer. The children's oral motor performance was evaluated during the feeding process using the Oral Motor Assessment Scale. Caries occurrence was also evaluated according the World Health Organization criteria. RESULTS: Motor skills were significantly associated with caries experience. Regarding the salivary parameters, osmolality presented a stronger association with caries experience than did the salivary flow rate. Children with worse oral motor performance presented a higher rate of caries occurrence. CONCLUSION: Osmolality exhibited a stronger association with caries occurrence than did salivary flow rate. This parameter, therefore, could be a potential caries risk indicator for spastic cerebral palsy children.

PURPOSE: The study aimed to assess electrocardiographic alterations during oral implant placement surgeries under local anesthesia (lidocaine chlorhydrate with epinephrine), using 15 mg of midazolam as an anxiolytic premedication. MATERIAL AND METHODS: The study randomly selected 20 patients, aged 21-50 years old, requiring bilateral mandibular dental implants. Each patient was assessed using placebo on one side and midazolam on the contralateral side, with random, double-blinded distribution. The electrocardiogram recorded 12 static leads every 2 min, while D2 derivations were recorded continuously. RESULTS: No statistically significant differences were observed between the placebo and midazolam when analyzing the morphological behavior of the electrocardiographic wave and the presence of arrhythmias during the experiment. However, under sedation, assessment of the behavior of electrocardiographic parameters during different stages of the procedure revealed statistically significant differences (P<0.05) for heart rate, P-wave amplitude and duration of the RR and QTc intervals. The arrhythmias detected were considered low risk for patients without systemic alterations and were observed in 53.3% of patients. The most frequently occurring alterations were tachycardia, bradycardia, supraventricular and ventricular extrasystoles and blocked atrial extrasystole, which were similar for both placebo and midazolam, with the greatest incidence during the initial, incision and bone drilling stages. CONCLUSION: The use of 15 mg of midazolam made no difference compared with the placebo. The use of 15 mg of midazolam did not show an advantage in the incidence of arrhythmias The anxiolytic premedication does not prevent myocardial arrhythmias in endosseous implant placement. The clinical significance of the arrhythmias may not represent serious risks.

RESUMEN

Drug-induced gingival enlargement has been reported in patients treated with various types of anticonvulsant drugs, and is generally associated with the presence of plaque, gingival inflammation, and a genetic predisposition. Effective treatment includes daily oral hygiene and periodic professional prophylaxis. However, in some patients, surgical removal of the gingival tissue overgrowth becomes necessary. The patient in this case report was mentally impaired and had severe drug-induced gingival enlargement. This report describes the initial protocol, the gingivectomy, and a 2-year follow-up. A diode laser was used as an effective and safe method to remove the patient's overgrown gingival tissue.

PURPOSE: The purpose of this study was to analyze electrocardiographic alterations during dental implant surgeries when local anesthetic agents were used. MATERIALS AND METHODS: Twenty implants were placed in 18 healthy patients. An electrocardiogram and Wincardio software were used to gather recordings from 12 static leads every 2 minutes, continuously record coronary artery (D2) derivations, and automatically measure the following electrocardiographic parameters: heart rate, duration and amplitude of the P wave, PR segment duration, ST segment deviation, QRS complex duration, and duration of the RR, QT, and corrected QT (QTc) intervals. RESULTS: Analysis of variance of the values obtained at the different stages showed significant differences (P<.05) for the heart rate and for the duration of the RR and QT intervals. The heart rate increased during the anesthesia, incision, and bone drilling stages, reaching a peak during drilling. Duration of the RR and QT intervals decreased during the incision and drilling stages. Among the electrocardiographic parameters individually assessed, several altered values were found for the duration of the P wave, the QRS complex, and the QT and QTc intervals. Sinusal tachycardia and bradycardia, sinusal arrhythmia, supraventricular extrasystole, ventricular extrasystole, and T-wave inversion were detected. CONCLUSION: Dental implant placement surgery may induce electrocardiographic alterations. The most frequently found arrhythmias were extrasystole and sinusal tachycardia. The anesthesia, incision, and bone drilling stages exhibited the highest heart rate values and the shortest durations of the RR and QT intervals.

A anemia ferropriva é uma anomalia freqüente na infância e decorrenteda deficiência de ferro, elemento essencial para a produção de hemoglobina.Manifesta-se clinicamente por palidez da pele, lábios e gengiva, queiliteangular, além de fadiga e fraqueza. O tratamento clínico é realizado com a administraçãode sulfato ferroso por via oral. Como efeito colateral, muitas vezespode ocorrer pigmentação negra nas superfícies dos dentes. A profilaxia dentalcom pasta profilática associada a pedra-pomes é indicada para a remoção dopigmento e, em alguns casos, a técnica de microabrasão pode ser necessária.


The iron-deficiency anemia is a frequentabnormality in childhood and it is resultingfrom a deficiency of iron, essential element for theproduction of hemoglobin. Clinically it is manifestedby paleness of the skin, lips and gums, angularcheilitis, as well as fatigue and weakness. Clinicaltreatment is prescribed, the medication to be takenorally is a substance that contains ferrous sulfate.Side effects occur often, one of them is a black orbrown pigmentation on the surface of the teeth.The dental prophylaxis with prophylactic pasteassociated with pumice stone is indicated for theremoval of such pigmentation, and in some casesthe technique of microabrasion may be needed. Inthis material are presented clinical cases of dentalpigmentation by ferrous sulfate.
